Equatorial Guineans travelling to Israel for medical treatment should apply for the Israeli visitor visa.
You need an original Equatorial Guinea passport with a minimum validity of 6 months from the date you plan to leave Israel for your visa application.
You must submit 2 recent, clear passport photographs (5x5 cm) taken with a white background.
A formal letter from a hospital stating the details of the treatment you plan to receive in Israel is required.
Proof of accommodation for the entire duration of your stay in Israel is required.
You must also submit a copy of your return flight ticket to Equatorial Guinea or your country of origin.
You are required to provide proof of travel insurance that covers the entire duration of your stay in Israel.
You must provide a document confirming the payment of your medical treatment.

A passport valid for at least 6 months from your departure date from Israel.
A completed and signed visa application form.
A photocopy of your passport and the visas in it.
2 recent passport-size photographs (5x5 cm).
A copy of your confirmed itinerary.
Proof of hotel accommodation for the entire duration of your stay in Israel.
Proof of financial means for the duration of your stay in Israel e.g. your bank statements of the last 6 months.
Proof of travel medical insurance that covers the duration of your stay in Israel.
A copy of your booked round-trip flight ticket.
Proof of payment of the visa fee.
