Equatorial Guineans travelling to Israel for medical treatment should apply for the Israeli visitor visa . You need an original Equatorial Guinea passport with a minimum validity of 6 months from the date you plan to leave Israel for your visa application. You must submit 2 recent, clear passport photographs (5x5 cm) taken with a white background. A formal letter from a hospital stating the details of the treatment you plan to receive in Israel is required. Proof of accommodation for the entire duration of your stay in Israel is required. You must also submit a copy of your return flight ticket to Equatorial Guinea or your country of origin. You are required to provide proof of travel insurance that covers the entire duration of your stay in Israel. You must provide a document confirming the payment of your medical treatment.

Foreign citizens who intend to travel to Germany for medical treatment require a short-stay visa for medical purposes. You must complete and sign the visa application form (category C) must be submitted. Invalid applications or incomplete visa application forms will not be accepted for processing and you will have to make a new appointment. 1 recent clear and coloured passport-size photograph (3.5 cm x 4.5 cm), with a bright background, must be submitted with your application form. You must submit your visa application form in person as your biometric information will be collected, unless in cases of serious illnesses. In the case of a serious illness, the consulate officer will make the final decision. A valid passport or other valid travel documents, with a clear photocopy of the first page, should be submitted. Your passport must be valid for at least 3 months after the end of your trip. A confirmed travel itinerary or a preliminary ticket booking must be presented. To avoid financial loss, you don’t have to buy a flight ticket. You are required to provide proof of accommodation in Germany for the entire duration of your stay. To show that you can finance your whole trip, including payment for the medical treatment and other related costs, you are to provide proof of financial solvency. This could be your recent payslips or your bank account statements for the last 3 months. You are also required to show proof of payment of the treatment. For proof of employment status: If you are an employee, you are to provide proof of your employment in the form of a signed and dated employment contract, showing your data and the contact details of your employer as well as a leave of absence letter and the date you are to return to work. If you are self-employed, you should submit proof of business certification such as the Business Licence and the company's certified bank account statements for the last three months. If you are retired, you should provide documents that prove that you are receiving a pension or other financial support. You are required to present a letter from the medical institution or doctor in Germany showing the following: proof of contact with your local doctor, nature of the treatment you’re travelling to Germany for, estimated time of recovery (and if there will be a need for return visits), the cost of treatment, an indication that the treatment can be performed in Germany and that the patient has been accepted. You must also submit a letter signed by your local doctor in Equatorial Guinea, not older than 3 months, providing background information about the medical treatment and confirming the need of specific medical treatment which will be provided in Germany.

An original passport that is valid for a minimum of 6 months. Two photographs taken against a white background.  Indian visa application fully completed and signed by the applicant. Typed Referral letter in original on letterhead with contact details, duly stamped and signed from a well-reputed medical institute after examination of the patient’s condition specifying the nature of the patient’s illness. The letter must also the reason for referring the patient for medical treatment to the established/specialized/recognized medical Institute/hospital concerned in India. Typed Invitation letter on letterhead, duly stamped and signed with contact details, from the hospital/medical Institute in India specifying the nature of the medical treatment to be undertaken along with the start and end date of this treatment. Detail of attendant/ attendants is to be indicated with the mobile number of the contact persons at the hospital. The applicant is to attach a printout of the invitation and the email page with the visa application form. A certified bank statement of last three months/guarantee letter issued by the bank in the name of the applicant, testifying the applicant’s ability to meet expenses to cover medical and living expenses in India for the duration of the medical treatment.  In case of sponsorship, an affidavit from the sponsor along with a copy of his/her Identity Card/Passport copy is to be attached.  In case of follow up treatment, previous medical papers and discharge summary from Indian hospital is to be attached to the application. Patients need to attach copies of their treatments in Equatorial Guinea including copies of CT Scan/MRI/X-ray reports. Transplant patients to produce a compatibility report of donor and patient. Indian hospital to undertake the responsibility of the patient and attendant including immigration till his departure from India. This undertaking is to be included in the invitation letter.

Applicants coming to the Netherlands for medical treatment must apply for a residence permit.  A letter to the Medical Advice Bureau in the Netherlands is needed. This letter should clearly state the nature of the illness, Why treatment elsewhere will be inappropriate and If you are capable of financing the treatment. A letter from your doctor in Equatorial Guinea should be submitted to the embassy. The letter should state the type of ailment and why treatment would be best gotten in the Netherlands. The hospital or doctor in the Netherlands will also need to send a medical attestation confirming the date of appointment as well as their knowledge of your medical situation. Applicants who are employed are to submit a letter from their employer stating the awareness of your leave and duration. Proof of financial sufficiency that covers the medical bills and duration of stay should be submitted. Proof of full or part payment of medical bills should be submitted. If the applicant is a minor, the original birth certificate along with a consent letter from one or both parents should be submitted. Equatorial Guineans on a medical visa are not allowed to work while they are in the country. Visa holders are not allowed to take up permanent residence in the country, but temporary residence is allowed. The applicant has to sign a medical declaration. By signing this declaration, the applicant consents to being involved in any medical research, if necessary. Applicants are expected to leave the country at the expiration of their or completion of their medical treatment.
