  • Copies of most recent bank statement showing proof of financial means.
  • Copies of confirmed hotel reservation as proof of accommodation in Poland.
  • Receipt showing that the application form has been paid.
  • A record of good character signed by the local police department in Equatorial Guinea is required.
  • A certificate of yellow fever vaccination is to be submitted by applicants.
  • All applications and supporting documents must be in English Language or Dutch.
  • Pets must be accompanied by a veterinary certificate issued by a registered veterinarian from Equatorial Guinea.
  • Children under 18 years of age must have a valid passport and letter of parental consent.
